Recently I received several PM’s asking about information that could have been easily located using Detective Forum’s search tool and reading prior post by other members. Namely information contained within the Social Security Death index (see URL above) and by ordering a copy of the SS-5 which is the Application for a Social Security Number. With a copy of this form you can learn more about people who died after or about 1960. As long as the person is deceased SSA will provide a copy under the Freedom of Information Act for a fee. They will also release this form to the registrant (the person who belongs to the Social Security Number) and to anyone who has a release-of-information statement signed by the person about whom the information is sought. Living persons have a right to privacy which includes the right not to have their Social Security Number revealed, but once a person dies they lose their right to privacy and therefore Social Security Administration can reveal their number and report their dates and places of birth and death. Keep in mind a small percentage of mistakes have been made on the list, some names omitted and other add inaccurately added. The request form can be printed from the above site.

The SS-5 usually includes the following:

Full name
Full name at birth, including maiden name
Present mailing address
Age at last birthday
Date of birth
Place of birth (city, county, State)
Father's full name
Mother's full name, including maiden name
Sex
Race as indicated by the applicant
Whether the applicant ever applied for Social Security or Railroad Retirement before
Current employer's name and address
Date signed
Applicant's signature
